  1. Click on the PDF form icon below

  3. If given the option, pick a location for the file
  4. Open the folder where the file was saved and double-click it to open.
  5. Print the form, fill it in, sign and date it.
  6. Have the form signed by PAC members who are your sponsors. If your sponsors are members of the Board of Directors, you may submit your application without signatures, and your sponsors will sign it at the next Board meeting.
  7. Mail the form, along with your check, to the address on the form, or give it to an Officer of the Division.
